BSCI审核需要提供的资料
1. Advice letter/工厂审核函：
Please kindly fill the actual total employee No.( including management) 请填写工厂现有用工总人数（包括管理层在内）
Should be signed by factory person & stamped with factory's company chop（Pls fill factory name and address in Chinese & English） 需工厂签名盖章扫描后邮件回传(请务必填写中英文工厂名字和地址)
2.BSCI self-assessment/BSCI自我评估表：
Pls fill in return and send back by email( It’s requested document by BSCI)请填写电子档邮件给我.( 此文件由BSCI所规定)
Please fill in the BSCI ‘self-assessment’, it is also required by BSCI. Please provide the information of your sub-contractors and key suppliers by filling the sheets of ‘production unit’ in the Self Assessment document, if you cannot or do not provide such information, please remark your reason in the document请将*分包商和主要供应商的资料填入自我评估表中的‘生产单位’页中，如果不能提供或不愿提供请将原因写入自我评估表中.
Subcontractor indicates business entity with appointed by the company to carry out value adding process steps of the assembling completely or partially (generally indicate those companies who are appointed by the factory/producer to take over entirely or partly the final production).分包商是指工厂指定用于实现全部或部分生产/装配程序的增值过程的商业实体（通常指整件产品的分包商或某些生产工序的分包商）。
Please copy the ‘production unit’ sheet for use if they are not enough in SA document.如果自我评估表中的‘生产单位’表不够用，可自行复制使。
3. Declaration sheet/同意声明书:
Should be signed by factory person & stamped with factory's company chop请工厂签名盖章扫描后邮件给我
4. Road map/路线图（从工厂所在市至工厂的大致路线）:
If no road map, please provide detailed routine or markable structure near your factory. 请提供工厂路线图，如果没有， 请手绘一份或提供具 体路线， 以便审核员能顺利到达贵厂。

请附上营业执照复印件、税务登记证复印件以及增值税一般纳税人资格证书复印件。否则，我们将开具增值税普通发票。
以个人名义银行转账付款给我公司，都不可以开具公司名称的增值税发票。只能开具缴款人个人名称的增值税普通发票。
